What to bring to your appointment

A government-issued photo ID (such as passport or driver’s license)

Form I-693 with your portion completed but DO NOT SIGN IT
What to Expect

Initial appointment to do a physical exam, review vaccination records and order labs and vaccines as needed

Return to pick up completed documents in a sealed envelope in one week or less
